• Please complete the membership form, enclose the $5.00 per person dues ($3.00 Teacher membership) and return it to your child’s teacher by October 6. Memberships received by September 15 will be recognized in the school directory. Payment is accepted in the form of cash or check. Make checks payable to Lincoln Heights PTA. Please write child’s name and teacher’s name on the check.
